I really liked the “Yuck Mouth” public service announcements that used to play on television when I was a kid.
  

“They call me Yuck Mouth 'Cause I don't brush I like my teeth like this I got some beef in my teeth and some chicken too... Ouch! That's a cavity! Hey! That's new! They call me Yuck Mouth 'Cause I don't brush, How 'bout a little kiss?!”

The commercial/cartoon obviously made an impression on me.  I can still hear the jingle and recall some of the images decades later.

Considering that I have a HUGE sweet tooth, my teeth are in pretty good shape: only one filling.

In high-school, one of my girlfriends accidentally chipped a small piece off one of my teeth.  I tickled her when she wasn't expecting it- she spun around really quickly, and her elbow hit me right in the mouth.  Fortunately, the chipped-off piece was so small, my tooth still looks completely normal.  I also had all four wisdom teeth extracted when I was in high school.
